● Please complete all fields of the form listed below with your $20 sitting fee for your child’s to be made.

● Only one sitting fee is required per family. In other words, if your child has siblings attending NCMA, you only need one payment of $20

● Your sitting fee is non-refundable, final and will NOT count to, or can be applied to the purchase of photography products.

● The last day to submit this form will be Monday, March 2nd.

● Please print out your completed form, sign, seal both pages in an envelope along with your payment deposit, and place in the drop box located at the front desk labeled “School Photos Drop Box”

● Please understand that signing and submission of this form is required for your child’s photos to be made. Without both, your child’s photos will not be taken.

Please read the contractual agreement below and sign. Your child’s photos will not be taken without a signature. Make Checks payable to “Senyo Kumassah” Returned checks will be assessed a $35.00 non-sufficient funds fee.

AGREEMENT:

This Agreement is made effective for all purposes in all respects of New Century Montessori Academy(NCMA) Fall 2019 School Photo by and between Senyo Kumassah, hereinafter referred to as "COMPANY" and Parents, hereinafter referred to as "CLIENT" relating to the event(s) detailed below, hereinafter referred to as "The EVENT(S)”.

ENTIRE AGREEMENT:This agreement contains the entire understanding between the COMPANY and CLIENT. It supersedes all prior and simultaneous agreements between the parties. The only way to add or change this agreement is to do so in writing, signed by all parties. In the event that any part of this agreement is found to be invalid or unenforceable, the remainder of this agreement shall remain valid and enforceable. Any agreement to waive one or more provisions of this agreement or any failure by one or both parties to enforce a provision of this agreement shall not constitute a waiver of any other portion or provision of this agreement.

MODEL RELEASE:The CLIENT hereby assigns the COMPANY the irrevocable and unrestricted right to use and publish photographs of the CLIENT or in which the CLIENT may be included, for editorial, trade, advertising, educational and any other purpose and in any manner and medium; to alter the same without restriction; and to copyright the same without restriction. The CLIENT releases all claim to profits that may arise from use of images. This release is not applicable to parents who have declined the release form established by NCMA.

PAYMENT AGREEMENT: The aforementioned $20 non-refundable sitting fee is due upon submission of the pre-order form by, Monday March 2nd, 2020, for participation in the Individual and Siblings’ session. The Client agrees to no refunds of the sitting fee if the Client so chooses not to purchase photos after proofs are submitted. All returned checks will be assessed a $35.00 non-sufficient funds fee.

PRICING: Services or merchandise not included in this initial contract will be sold at the current price when the order is placed. All prices are subject to change at any time without notice. Credit vouchers have no intrinsic cash value and may only be applied toward merchandise purchased from the COMPANY.
